How your body regulates testosterone

To understand how testosterone works, it helps to think of your body like a central heating system. The thermostat represents your hypothalamus and pituitary gland, while the boiler represents your testes.

When your body senses that testosterone levels are too low, the hypothalamus releases a signal called gonadotrophin-releasing hormone (GnRH). This signal tells the pituitary gland — the control centre — to release two important hormones: LH (luteinising hormone) and FSH (follicle-stimulating hormone).

These hormones travel to the testes, stimulating testosterone and sperm production. This natural process keeps your testes active, healthy, and functioning properly. The Leydig cells inside the testes are responsible for producing testosterone and are highly sensitive to the LH signal from the pituitary gland.

However, when external testosterone is introduced — for example, through testosterone replacement therapy (TRT) or anabolic steroid use — this natural signalling process can slow down or even shut off completely. Without stimulation from LH, the Leydig cells may stop producing testosterone on their own, which can lead to testicular shrinkage (atrophy) and reduced fertility.

To counter this effect, HCG (human chorionic gonadotrophin) or recombinant LH and FSH can be used alongside TRT. This type of adjunctive therapy helps maintain testicular function and sperm production, allowing men to protect fertility while undergoing testosterone replacement therapy.

 

Age-Related Testosterone Deficiency

As men get older, testosterone levels naturally decline — a condition known by several names, including:

  • Andropause
  • Manopause
  • Adult-onset hypogonadism
  • Male menopause
  • Low T (low testosterone)

Clinically, testosterone deficiency can be classified as:

  • Primary hypogonadism – when the testes themselves are not functioning properly

Diagnosing Low Testosterone

When diagnosing low testosterone (low T), doctors look at LH (Luteinising Hormone) and FSH (Follicle-Stimulating Hormone) levels to identify whether the issue is in the testes or the brain.

Secondary hypogonadism occurs when the hypothalamus or pituitary gland fails to send the right signals, leading to low LH, low FSH, and reduced testosterone production.

Primary hypogonadism happens when those signals are normal, but the testes don’t respond properly, resulting in high LH and FSH but still low testosterone.

As testosterone levels rise, some is converted to oestrogen through the aromatase enzyme. Higher oestrogen tells the brain to reduce testosterone and sperm production, which over time can lead to testicular shrinkage (atrophy).

Using testosterone replacement therapy (TRT) without managing oestrogen or supporting the testes with HCG can worsen atrophy and lower natural testosterone production. A well-balanced TRT plan includes oestrogen control, HCG support, and regular blood monitoring to maintain hormonal balance and wellbeing.

  • Nebido® (Bayer) – Testosterone undecanoate 250 mg/mL in a 4 mL ampoule (1 g total dose), typically injected every 10–14 weeks.
  • Testosterone Enanthate – A long-acting testosterone ester (250 mg/mL), usually administered two to three times per week.
  • Testosterone Cypionate – Another long-acting ester popular in TRT, typically dosed two to three times per week for stable blood levels.
  • Virormone® (Nordic) – Short-acting testosterone propionate (50 mg/mL), generallyadministered multiple times per week.

  • TOSTRAN® (ProStrakan) – 2% testosterone gel (10 mg per metered application), supplied in a 60 g multidose dispenser.
